Overview Physical Therapist needed for a travel assignment in a Skilled Nursing Facility in Monterey, California. Responsibilities

• Conduct initial and ongoing physical therapy evaluations to identify functional deficits and establish goals for residents in the SNF.
• Develop and implement individualized treatment plans focusing on mobility, strength, balance, and endurance; adjust plans as needed based on progress.
• Provide direct interventions including therapeutic exercises, gait training, transfers, and balance training; apply safe, evidence-based modalities as appropriate.
• Educate residents and families on home exercise programs, safety strategies, fall prevention, and discharge planning.
• Collaborate with an interdisciplinary team (nursing, occupational therapy, speech-language pathology, physicians) to coordinate care and document progress in charting and electronic records.

Requirements

• Active Physical Therapist license in California or eligibility to obtain licensure in California.
• Doctor of Physical Therapy (DPT) degree from an accredited program.
• Basic Life Support (BLS) certification.
• Experience with geriatric populations in a skilled nursing facility or long-term care setting preferred.
• Strong communication and teamwork skills; ability to document and maintain accurate records.
