Integrated Care Manager - LTSS & Waiver Services

Molina Healthcare of Illinois

Molina Healthcare is seeking a Care Manager to support LTSS care coordination and interdisciplinary teams in Stewardson, Illinois. The role focuses on assessing needs, developing and monitoring care plans, and coordinating waiver services to improve member outcomes while controlling costs.
