Virtual NP - Primary Care & Home Health Lead

Upward Health in California seeks a Nurse Practitioner to deliver high-quality, patient-centered primary care primarily through our Facilitated Virtual Care platform, with some in-person visits to support transitions of care and wellness visits. You will collaborate with a multidisciplinary team to manage complex chronic conditions, perform risk assessments, and coordinate care transitions while participating in after-hours coverage for our 24/7 patient hotline in a home-based care model. #J-18808-Ljbffr Upward Health
