Physical Therapist

As a Physical Therapist, youll provide high-quality, compassionate care to patients in their homes, promoting Elara Carings mission to deliver the Right Care, at the Right Time, in the Right Place. You will evaluate functional needs and outcomes, design and implement individualized care plans, and collaborate with interdisciplinary teams to ensure patient progress and satisfaction.

• Promote Elara Carings philosophy, mission, and policies to ensure quality of care.
• Provide direct patient care and assess functional outcomes.
• Design and implement individualized plans of care.
• Direct support staff to achieve functional goals.
• Document evaluations, treatment goals, and maintain prescriptions and signatures.
• Provide leadership to promote physical therapy in the community.
• Work within physical demands of the role, including lifting and transferring patients.

Requirements:

• Graduate of an American Physical Therapy Association (APTA) approved program.
• Current, unrestricted PT license in the state of employment.
• Minimum one year of experience in a healthcare facility.
• Valid drivers license, dependable vehicle, and auto insurance.
• Ability to travel within branch coverage area (up to 50%).
