Physical Therapist Case Manager: Home Health

Please note: All opportunities at 21HHS require being in the field visiting patients in their homes. Full Time Physical Therapist Case Manager Anticipated Territory: Folsom Area In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment. Your goals as a Physical Therapist are to restore function to prevent or minimize disability following disease or injury and to help the client reach his/her maximum performance/function within the limits of his/her capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs and may also supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. Responsibilities Perform an evaluation in the home, develop a care plan and work with PTAs to follow the care plan. Help patients with mobility, movement, and support in regaining their independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure a quality level of care. Evaluate and report patients’ progress to the MD. Assist with documentation and administrative tasks as part of Concierge Service. Create flexible work schedules; you can design your own schedule and hours. Work in targeted territories (zip code based assignments) to reduce driving time. Operate with autonomy—no clocking in/out. Maintain a low turnover/high morale environment. Use company-provided software and communication tools. Receive a company-issued device (iPad or Laptop). Use a company-issued phone number. Maintain an annual membership to MedBridge for CEU training and patient-facing HEP. Enjoy mileage/vehicle reimbursement of $0.70 per mile between visits. Competitive compensation with an achievable incentive bonus program. Qualifications Current unrestricted California Physical Therapist License. Current BLS/CPR Certification. Valid California Driver’s License. Knowledge of local community resources. Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury and the influence of culture on health care. Knowledge of state and federal home‑care regulatory guidelines preferred. Experience with home health care highly preferred. Experience with high-acuity patients is a plus. Experience working with an interdisciplinary team highly desired. Case Management fundamentals highly desired. Self‑directed. Empathetic. Critical thinker. Outstanding interpersonal skills.
