Direct Support Professional

Major Responsibilities:

• Identification and implementation of intervention to address barriers that impeded the development of skills necessary for independent function in the community.
• Obtaining input from the consumer/providers/significant others about the service delivery process and seeking information from anyone in an effort to obtain needed services/supports on behalf of the person.
• Informing significant others about the consumer's situation and the care coordinator's efforts on behalf of the other person with the consent of the person/legally responsible person.
• Locating and coordinating sources of help from within the family and community so that the individual receives available natural and community support. Completing application forms to assist in receiving community and other formal service support.
• Monitoring the consumer's situation to assure quality care as well as the continued appropriateness of the services including review of documentation of providers, providers' claims, and evaluation/progress summaries of providers. This includes observation of service delivery.
• Observing the consumer's education services, including attending ISP meetings and school transition meetings, and referring/linking to services.
• Therapeutic Mentoring, Symptoms monitoring, monitoring medications, and self-management symptoms.
• Communicate relevant consumer information to supervisors, care coordinator, and/or parents/guardians.
• Provide privacy and confidentiality of all consumer records.
• Assure consumers are free from abuse, mistreatment, and or neglect and report any incidents of such to supervisor.
• Treat each consumer with dignity and respect.
• Complete all documentation related to Treatment Plans.
• Develop and demonstrate a thorough understanding of the philosophy and purpose of the agency.
• Develop familiarity with the Treatment Plan (ISP) of each consumer served.
• Implement therapeutic goals and recommendations and follow appropriate documentation procedures.

Direct Support Provider Job Description
• Provide a positive atmosphere which facilitates growth and learning of coping skills.
• Make decisions based on training and policies when providing services.
• Perform duties as requested by Supervisor.
• Other duties as assigned

• Thorough understanding of individual record documentation requirements
• Must be at least 18 years old
• High School Diploma or equivalent
• Valid driver's license, current automotive insurance, and reliable transportation
• Ability to pass criminal record, driving record, and healthcare registry checks
• Lifting may be required depending on individual client needs

Education & Experience: GED or high school diploma
