Key Responsibilities\n

• Collaborate with an interdisciplinary health care team to develop comprehensive treatment plans, including establishing effective discharge plans based on detailed psychosocial assessments and evidence-based social work methodologies.

• Conduct thorough patient assessments and provide appropriate interventions, maintaining accurate and timely clinical documentation, including session notes and progress reports, in compliance with department policies.

• Deliver crisis intervention services, using your expertise to assess and address emotional and behavioral issues presented by patients during treatment.
