Job Requirements

• Active California Licensed Practical Nurse (LPN/LVN) license
• Current BLS certification
• Strong assessment, documentation, and communication skills
• Ability to work independently and collaboratively with the care team
• Comfortable caring for residents with varying physical and cognitive needs
• Professional, dependable, and adaptable with a strong patient-centered approach
• Ability to meet the physical demands of patient care, including walking, bending, and lifting

Responsibilities

• Provide direct nursing care to residents in a long-term care setting
• Complete patient assessments and report changes in condition promptly
• Administer medications and treatments accurately and safely
• Document nursing care, assessments, and resident responses in a timely manner
• Assist with procedures, specimen collection, EKGs, blood draws, and IV starts as needed
• Support resident comfort, safety, and dignity throughout the shift
• Maintain infection control and safety standards at all times
• Collaborate with RN leadership, physicians, residents, families, and other departments
• Respond appropriately to urgent situations and initiate emergency measures when needed
• Contribute to a positive team environment and deliver excellent resident-centered care
