Humana's Primary Care Organization is seeking a full-time Primary Care Nurse Practitioner (NP), or Physician Assistant (PA) to join our team at our clinic in Jacksonville, FL.

As a Nurse Practitioner (NP) or a Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, diverse team. This team includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

You will report to the Associate Medical Director, and work at our clinic located at Phillips Hwy, FL.

Your Role & Responsibilities

• You will provide comprehensive care to seniors following clinical standards.
• You will collaborate daily with your care team to ensure coordinated, high-quality care.
• You will manage referrals, hospital/SNF coordination, DME, and home health services.
• You will document care with support from quality-based coders.
• You will participate in on-call rotation and support growth plans at your center.
• You will have experience with providing value-based care.
• You will work on-site.

What We're Looking For

Required:

• Master's degree in nursing with board certification.
• Active, unrestricted NP, or PA license in the state of Florida.
• Experience in value-based care, primary care or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Scheduled Weekly Hours: 40

Pay Range: $123,800 - $170,300 per year. This job is eligible for a bonus incentive plan.
