Estimated pay package of $2,566/week.\n \n Home Health Physical Therapist position. Coverage throughout the Cleveland/Solon service area. Responsible for evaluations, treatment planning, mobility training, home safety assessments, equipment recommendations, and patient/family education. Adult and geriatric patient population. Expected productivity: approximately 30 units per week. Home Health experience preferred. Homecare Homebase (HCHB) experience preferred. OASIS experience preferred. Strong clinical support and leadership team. Opportunity to work independently while making a direct impact in patients' homes. Weekly pay package calculated and confirmed prior to submission.
