About Us Care Connect at Home is a family-owned and operated home care agency serving Palm Beach, Martin, St. Lucie, Indian River, and Okeechobee counties. We started this agency after experiencing firsthand how difficult it was to find compassionate, reliable care for our own grandmother. Our mission is simple: provide exceptional care while building lasting relationships with families and referral partners. Position Summary We're looking for a motivated Community Liaison to help grow Care Connect at Home throughout Palm Beach County and the surrounding areas. This role is perfect for someone who enjoys meeting people, building relationships, and making a meaningful impact in the community. You'll be responsible for developing referral partnerships with hospitals, physicians, rehabilitation centers, assisted living communities, senior living communities, case managers, social workers, and other healthcare professionals. Responsibilities Build and maintain referral relationships throughout the community Visit hospitals, rehab centers, ALFs, ILFs, physician offices, and senior communities Conduct regular in-person sales calls and presentations Generate qualified referrals and new client opportunities Attend networking and community events Follow up with referral sources and prospective clients Track activities and referrals in our CRM Help grow the Care Connect at Home brand within the community Qualifications 2+ years of healthcare sales or community liaison experience preferred Home care, hospice, home health, or senior living experience strongly preferred Excellent communication and relationship-building skills Self-motivated and organized Comfortable working independently in the field Reliable transportation and valid driver's license Compensation Competitive base salary + uncapped commission (based on experience) Commission-only option available for experienced candidates Mileage reimbursement Flexible schedule Opportunity for advancement as our agency continues to grow #J-18808-Ljbffr
