Peer Community Health Navigator for Housing & Reentry
St Johns Community Health
St. John’s Community Health seeks a Peer Community Health Worker in Los Angeles to provide services that improve lives through culturally relevant connections to healthcare and housing-related resources. The role relies on lived experience and collaboration with Case Managers and Housing Navigators to address Social Determinants of Health. The CHW will support housing applications, document management, workshops, and program reporting, while maintaining confidentiality and trauma-informed care #J-18808-Ljbffr St Johns Community Health
