Case Management Nurse

Inpatient Care Manager (RN) – Contract Location: New York, NY 10004 Contract Duration: 13 weeks ⏰ Schedule: 9am - 5pm (Rotating weekends) Job Type: Contract Pay Rate: $55/hr on W2 Job Overview We are seeking an experienced Inpatient RN Care Manager to support hospitalized patients through safe, efficient, and timely transitions of care. This role focuses on coordinating clinical, psychosocial, and financial aspects of care while collaborating closely with interdisciplinary teams to optimize patient outcomes and resource utilization. Key Responsibilities Coordinate inpatient care plans to support timely and safe discharge planning Conduct clinical assessments and collaborate with providers to align care goals Identify high-risk patients and facilitate referrals to social services Implement strategies to reduce readmissions and address barriers to discharge Monitor length of stay and participate in interdisciplinary care rounds Coordinate transitions across levels of care throughout the continuum Communicate payer, authorization, and financial considerations impacting care Educate patients, families, and care teams on care progression and resources Ensure compliance with federal and state regulations, including CMS and Medicare guidelines Identify and escalate quality, risk management, or compliance concerns as needed Required Qualifications Active RN license Minimum 2 years of inpatient care management or acute care experience Strong communication, negotiation, and care coordination skills Proficiency with clinical documentation and healthcare systems Preferred Qualifications Case Management Certification Prior travel assignment experience Background in acute inpatient case management Why Join Pride Health Pride Health offers eligible employees comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance, and hospital indemnity), 401(k) retirement savings, life & disability insurance, an employee assistance program, legal support, auto and home insurance, pet insurance, and employee discounts with preferred vendors.
