As a registered nurse, you will be responsible for assessing, planning, coordinating, and overseeing patient care while also providing supervision and guidance to caregivers. This position demands strong clinical expertise, exceptional communication skills, and a commitment to promoting patient well-being and caregiver competence. *Key Responsibilities:* Patient Assessment: Conduct comprehensive initial and ongoing assessments of patients' health status, including physical, emotional, and psychosocial needs. Care Planning: Develop individualized care plans in collaboration with patients, their families, and the interdisciplinary healthcare team, addressing specific needs and goals. Coordination of Care: Coordinate and facilitate the provision of healthcare services, therapies, and medical equipment as prescribed in the care plan. Patient Education: Provide patients and their families with education on their medical condition, treatment options, and self-care techniques to enhance their independence and well-being. Medication Management: Administer medications and monitor medication adherence, side effects, and interactions, ensuring patient safety. Clinical Documentation: Maintain accurate and up-to-date patient records, documenting all assessments, interventions, and changes in the patient's condition in compliance with regulatory standards. Monitoring and Evaluation: Regularly evaluate the patient's progress towards their goals and make necessary adjustments to the care plan as needed. Advocacy: Serve as an advocate for patients and their families, ensuring their needs and preferences are effectively communicated and met within the healthcare system. Team Collaboration: Collaborate with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to provide comprehensive and holistic care. Caregiver Supervision: Provide guidance, training, and supervision to caregivers, ensuring they are competent and capable of providing quality care to patients. Quality Assurance: Participate in quality improvement initiatives to enhance the delivery of care and maintain compliance with regulatory standards.
