Travel Home Health Physical Therapist

This role is a 13-week contract for a local travel home-health physical therapist based near Springfield, Missouri. The therapist will conduct comprehensive evaluations, develop individualized treatment plans, and manage OASIS documentation to improve patient mobility and independence. The position offers competitive weekly compensation and flexible travel opportunities.

Key Responsibilities:

• Conduct comprehensive patient evaluations in the home health setting
• Develop individualized treatment plans focused on improving mobility, strength, balance, and independence
• Perform start of care assessments, recertifications, and routine home health visits
• Complete accurate OASIS documentation and maintain compliance with home health regulations
• Serve as a case manager coordinating care with physicians, nurses, therapists, and interdisciplinary teams
• Educate patients and caregivers on therapeutic exercises, mobility strategies, and safety precautions
• Monitor patient progress and modify treatment plans as needed
• Document all patient interactions and treatment outcomes using Matrix EMR
• Deliver compassionate, patient-centered care while promoting functional independence and quality of life

Qualifications:

• Doctorate or Master's degree in Physical Therapy from an accredited program
• Active Missouri Physical Therapist license
• Strong experience with OASIS documentation, case management, start of care assessments, and recertifications
• Excellent communication, organizational, and time-management skills
• Ability to work independently in a home health environment

Preferred Skills:

• Current CPR/BLS certification
• Home health experience highly preferred for travel assignments
• Experience with Matrix EMR
