Job Title: Home Health Aide
Job Description:
Salary: $20/hr

Minimum Qualifications:
- Current New York State Home Health Aide (HHA) certification
Job Duties:
- Escort residents to and from clinic appointments and dialysis sessions
- Monitor and prevent falls and altercations, intervening immediately
- Assist residents in preparing for appointments and ensure they receive snacks during out-on-pass periods
- Report incidents and accidents promptly to the clinical coordinator or ADNS
- Act as a representative of the facility, adhering to all policies and procedures on and off the property
