Registered Nurse (RN) Case Manager – PRN Home Health

Dallas County | Dallas–Fort Worth & Surrounding Areas

Grace Unlimited Home Health Care is seeking experienced  PRN RN Case Managers to provide high-quality home healthcare to clients primarily in  Dallas County , with coverage in surrounding Dallas–Fort Worth areas as needed. This role is ideal for nurses seeking  flexible, PRN work while delivering compassionate, patient-centered care in the home.

Role Responsibilities:

• Provide professional, compassionate nursing care in clients’ homes

• Perform assessments and manage individualized care plans

• Accurately document care using  OASIS/Kinnser

• Manage diverse patient needs in a home health setting

• Respond to PRN and urgent visit needs as assigned

Qualifications:

• Graduate of an accredited nursing program

• Active Texas RN License

• Valid Driver’s License and reliable transportation

• Minimum 1 year of home health experience

• Experience with  OASIS/Kinnser preferred

• Ability to travel locally within  Dallas County

• Flexible availability for PRN visits

Work Location:

• Client homes throughout  Dallas County

• Field-based / on-the-road role
