Community Health Worker

Community Health WorkerWorking closely with community agencies, social workers and the Market-Based Population Health teams, at Privia Health, the Community Health Worker will provide assessment of patient needs, make community referrals, and follow up with patients who are identified as benefiting from additional support and intervention from their health care team.Minimum Qualifications:Education: High School DiplomaLicensure: Certified Community Health Worker (within 1 year of hire date) Company will provide training optionsExperience: Minimum of 1 year of experience in social service or healthcare settingExperience working with at-risk patients is highly desirable.Skills: Strong communication, and interpersonal skills.Ability to work in a fast-paced environment and manage multiple priorities.Proficiency in electronic health record (EHR) systems.Competencies: Empathy, critical thinking, and the ability to work collaboratively with diverse populations and interdisciplinary teams.Interpersonal Skills & Attributes:Strong interpersonal skills.Excellent communication, interpersonal, and organizational skills.Ability to work collaboratively in a multidisciplinary team environment.Proficiency in electronic health records (EHR) and care management software.Knowledge of community resources and social service systems.Demonstrate sound judgment and strategic mindset through critical decision-makingMaintain a positive, caring attitude toward patientsAbility to function efficiently and meet defined goalsMust be detail-orientedPossess a focused work ethic and a high level of professionalismMust be able to travel 1-2 times a month to Stephenville, Midlothian or EnnisSelf-motivated: Identifies opportunities and finds solutions to problemsAble to multitask effectivelyProficient with Microsoft Office applications including Word, Excel, and PowerPoint as well as Google Docs
