Banyan Treatment Centers is seeking a dedicated Aftercare Coordinator to support patients as they transition through every stage of treatment. This on-site role focuses on discharge planning, continuity of care, and collaboration with clinical staff, families, and external partners to ensure each patient leaves treatment with a clear, well-coordinated aftercare plan. The coordinator will manage documentation in the EMR, use Teams and Salesforce for coordination, and engage families to support #J-18808-Ljbffr Banyan Treatment Center
