Physical Therapist

Key Responsibilities:

- Evaluate residents' physical abilities, mobility limitations, and rehabilitation needs.
- Develop and implement individualized treatment plans based on physician orders and patient goals.
- Provide therapeutic exercises, gait training, balance training, and mobility interventions.
- Monitor patient progress and adjust treatment plans as needed to maximize outcomes.
- Educate residents, families, and caregivers on exercises, safety techniques, and injury prevention.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to coordinate care.
- Maintain accurate and timely documentation of evaluations, treatments, progress notes, and discharge summaries.
- Ensure compliance with facility policies, regulatory requirements, and professional standards of practice.
