Job Title: Youth Services Coordinator
Department: Caritas Youth Therapeutic Treatment Center
Status: Full-Time, Exempt
Reports To: Service Coordination and Aftercare Supervisor
Starting Compensation Pay : $52,416 annually
PURPOSE: The Youth Service Coordinator position is responsible for ensuring the coordination and provision of all residential based case management services. This position will work as the liaison between St. John Bosco Children’s center staff and the case carrying agency of all youth admitted to SJBCC; will work to ensure that all requirements of the Family First Act are provided and followed.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Serves as the primary point of contact for all communication for assigned youth.
- Coordinates, schedules, and leads staffing’s (pre-admission, admission, monthly, discharge, and aftercare) for all assigned youth. Completes requires CSR documentation within 24 hours in the state RedCap system.
- Coordinates all school related activities for assigned youth, to include enrollment, attendance to IEP meetings, and all necessary communications with schools.
- Advocates for the educational, medical, social, emotional, and physical needs of the assigned youth to the various members of youth’s teams.
- Coordinates and schedules all visits/passes for assigned youth.
- Documents all client related communications within the ACE database.
- Completes all required 906 documentation for assigned youth within the required timelines.
- Attends phone conferences and training to keep current on State requirements and best practice in the field.
- Completes documentation related to Medicaid service provision in a manner that is compliant with Medicaid billing standards.
- Provides direct services and crisis intervention work to residents in daily activities as needed.
- Attends weekly supervision with Residential Service Coordination Supervisor.
- Perform additional duties as assigned.

Requirements:QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must demonstrate the ability to coordinate services for youth in residential treatment, collaborate with multidisciplinary teams, advocate for youth needs, and maintain compliance with Family First Act, Medicaid, and state documentation requirements. The individual must be able to exercise sound judgment, maintain confidentiality, and effectively communicate with youth, families, schools, case-carrying agencies, and community partners. EDUCATION and/or EXPERIENCE REQUIRED/PREFERRED:
Degree/Field Required: Bachelor's Degree in Social Work or related Human Services field

- Must qualify as a Mental Health Professional (MHP) under Illinois Department of Mental Health Rule 132.

Social: Ability to establish and maintain effective working relationships with youth, families, case-carrying agencies, schools, service providers, community partners, and residential staff. Demonstrates professionalism, advocacy, collaboration, and effective interpersonal communication skills.
Problem Solving/Decision-Making: Ability to assess youth needs, identify barriers to successful treatment and discharge planning, coordinate resources, exercise sound judgment, and make timely decisions that support youth safety, well-being, and permanency goals.
Proficient Computer Skills In: Excel, Outlook, Word, database management software, electronic documentation systems, and state reporting platforms.
Expert Computer Skills In: N/A
Resource Management: Ability to coordinate staffing meetings, educational services, visitation schedules, discharge planning activities, community resources, and multiple case responsibilities while ensuring compliance with required timelines.
Technical: Ability to maintain Medicaid-compliant documentation, complete required state reporting, coordinate educational and treatment services, manage case records, facilitate team meetings, and utilize agency databases effectively.
Trauma-Informed: Must display a willingness to adopt trauma-informed interventions, principles, and practices and demonstrate a commitment to ongoing professional development in trauma-informed care. Recognizes the impact of trauma on youth, families, staff, and communities and incorporates trauma-informed principles into service coordination, advocacy, treatment planning, and family engagement activities.
