Compassionate Hospice RN Case Manager

Gentiva Hospice is seeking an experienced RN Case Manager to deliver compassionate in-home hospice care. You will assess patients, develop individualized plans of care, and collaborate with an interdisciplinary team to ensure comfort and dignity for patients and families. Primarily home visits with documentation, education for caregivers, and adherence to regulatory standards. CPR certification and valid driver’s license required. #J-18808-Ljbffr Gentiva
