Registered Nurse - Home Health

This is a 13-week contract position.

AM shifts, 5 days a week (Monday to Friday), 8 hours per shift.

We are seeking a dedicated Registered Nurse (RN) with recent home health experience to join our team. The ideal candidate will have proficiency in HomeCare HomeBase and will provide nursing care to patients in their homes while covering Rankin County and surrounding areas as needed. This role may require travel within a 60-mile radius, and participation in various patient care activities is essential.

Responsibilities:

• Conduct patient assessments and develop care plans.
• Execute start of cares, resumption of cares, recertifications, and discharges.
• Perform routine visits and document nursing interventions accurately.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Meet the minimum requirement of 30 points through various patient care activities.

Required Experience / Certifications / Licensure:

• Active RN license.
• Recent experience in home health care.
• HomeCare HomeBase experience required; candidates without this experience may be considered at a reduced rate for training purposes.
• Willingness to obtain necessary continuing education points as required.
