Self-Direction PPP Support Counselor (Field Based - Burlington County)

About the Role The Self‑Direction PPP Support Counselor conducts quarterly in‑person evaluations for members enrolled in the Personal Preference Program in Monmouth or Ocean Counties. The counselor manages workforce functions such as budgeting, backup plan development, capacity planning, forecasting, safety planning, scheduling, and real‑time monitoring. They provide member assistance and strategic insight while reviewing and updating cash management plans. What You'll Do Work with a member to develop a member‑specific cash management plan for daily needs. Review monthly cash expenditures to ensure members stay within the monthly budget allowance. Conduct quarterly budget reviews and update plans as required. Develop and review the member’s backup plan each quarter. Analyze worker timecards for PTO submissions, EVV compliance, and hours of work. Update and maintain the electronic medical record for each member. Identify, report, and follow up on suspected fraud, waste, neglect, or abuse. Locate and maintain community resources and other services. Document in‑home visit notes and upload them on the day of the visit. Educate members on notification requirements for condition changes, hospitalizations, or caregiver issues. Provide education on safety tips (e.g., fall risk prevention). Conduct clear and consistent communication. Perform additional tasks as assigned. What You Bring High School Diploma or GED (required); LPN or healthcare background preferred. Equivalent experience in budgeting or finance preferred but not required. Minimum of 3 years’ experience in a health insurance or professional business‑related field. Proficiency with MS Office and other workforce management solutions highly preferred. Knowledge of the healthcare industry helpful. Ability to balance multiple priorities and strong analytical, interpersonal, and communication skills. Proficient use of MS Word, Excel, and PowerPoint. Skills and Abilities Demonstrated strong analytical skills with emphasis on forecasting. Problem solving, judgment, critical thinking, and decision making. Highly organized, detail oriented, accurate, and timely. Ability to work independently and with team, including under time pressure. Willingness and ability to learn new software applications. Ability to visit diverse populations in their homes, including varied home settings. Ability to assume an increased home visit schedule (greater than 60% of working hours). Efficiently organize home visit caseload to maximize driving time.
