Medical Social Worker

Conduct patient assessment interviews Provide patient and family referrals and helpful resources Coordinate and implement complex discharge plans Provide patient education and supportive and crisis interventions Perform bio-psychosocial assessments and evaluate patient and caregiver needs Collaborate and communicate with multidisciplinary teams through verbal communication and medical-record maintenance Maintain patient records and document critical information for the medical team Maintain professional and technical knowledge and required certifications through continuing education and development Utilize management resources Participate in an on-call rotation Adapt behavior and communication to the specific patient population Remain knowledgeable about applicable federal, state, and local laws, accreditation standards, and regulatory requirements Report known or suspected unethical, questionable, safety, privacy, or compliance-related concerns Requirements Master's degree in Social Work (MSW) from a university accredited by the Council on Social Work Education (CSWE) No work experience required Certified Social Worker (CSW) as verified by the LABSWE OR Registered Social Worker (RSW) certificate Computer skills and dexterity for data entry and retrieval Proficiency with Windows-style applications, keyboard, and role-specific software packages Effective verbal and written communication skills Working knowledge of common disease entities, disorders, and medical terminology Working knowledge of acute and chronic care medical settings Knowledge of healthcare systems and public programs including Medicare, Medicaid, Medicare Advantage, and commercial insurance plans Ability to work a flexible schedule, including 24/7, weekends, holidays, and on-call availability Ability to sit and stand for prolonged periods Ability to travel throughout and between facilities Ability to meet Medium Work physical demands and use mechanical lifting devices or team lifts Ability to work in a healthcare environment with potential exposure to communicable diseases, hazardous medications, or hazardous waste Core Competencies Demonstrates expertise in conducting patient assessments, coordinating discharge plans, and providing education and crisis interventions.
