Position Summary

As a Physical Therapist , you will assess, plan, organize, and deliver rehabilitative care designed to improve mobility, reduce pain, restore function, increase strength, and help patients regain independence following illness or injury.

This role offers the opportunity to build meaningful patient relationships while delivering individualized care where patients are most comfortable—their homes.

What You’ll Do

• Perform and document comprehensive initial evaluations and establish treatment diagnoses prior to intervention.
• Develop and implement individualized treatment plans to improve or restore physical function.
• Evaluate patient progress and modify treatment plans to maximize outcomes.
• Educate patients and families on therapeutic techniques and recovery goals.
• Apply infection control and universal precaution standards during patient care.
• Ensure continuity and quality of care through timely and accurate documentation.
• Communicate effectively with interdisciplinary teams and healthcare professionals.
• Support agency values, mission, and operational standards.
• Maintain all clinical documentation within company-established guidelines.

Qualifications

• Graduate of an accredited Physical Therapy program
• Current, active Physical Therapist license
• Minimum 1 year of Home Health experience preferred
• Current CPR Certification
• Valid Driver’s License and Auto Insurance
• Experience with HomeCare HomeBase (HCHB) preferred

Physical Requirements

• Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods
• Ability to lift, carry, push, and pull up to 100 lbs.
