Territory Clinical Manager (TCM)

We are seeking an experienced Territory Clinical Manager to oversee clinical and office operations while ensuring the delivery of high-quality home health services. The ideal candidate will provide leadership, maintain regulatory compliance, support clinicians, and drive quality improvement initiatives to ensure exceptional patient care. Key Responsibilities Supervise daily clinical and office operations and remain available to staff and patients during business hours. Ensure services comply with professional standards, agency policies, and state/federal regulations. Coordinate patient referrals and collaborate with the interdisciplinary care team. Oversee development and implementation of individualized patient care plans. Monitor quality of clinical care and continually assess patient needs. Collaborate with physicians and clinicians regarding patient transfers, discharges, and changes in condition. Manage OASIS documentation, edits, discharge processes, and related data. Track key performance indicators (KPIs), workflows, and escalations. Guide clinicians on emergency care protocols. Provide training, mentoring, and supervision to clinical staff, including contract clinicians. Lead Quality Assessment and Performance Improvement (QAPI) activities, investigate incidents, analyze trends, and implement corrective actions. Support accreditation readiness, quality audits, HHCAHPS surveys, and emergency preparedness planning. Serve as a backup for leadership roles and act in the absence of the Territory Administrator. Collaborate with the team to support exceptional home health care services. Qualifications Graduate from an accredited school of professional nursing. Active Registered Nurse (RN) license in the state of agency operation. 3–5 years of RN experience required. Experience managing RN services in home health, community, or clinic settings preferred. Strong leadership, communication, and organizational skills. Required Certifications & Licensure Current RN license in the state of practice. Valid CPR certification.
