Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments tailored to each patient, ensuring that evaluations are culturally responsive and informative for the treatment transition within the primary care setting.

• Deliver a range of behavioral health services, including counseling, individualized treatment plans, crisis intervention, and support for substance abuse issues in both individual and group settings.

• Perform detailed documentation of mental health evaluations, progress notes, and crisis assessments electronically to maintain seamless communication with the healthcare team.

Qualifications\n
