Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the psychosocial needs of senior clients, considering emotional, mental, and social factors that affect their well-being.

• Facilitate effective communication among family members involved in the senior client’s care, helping resolve conflicts and misunderstandings.

• Educate clients and their families about available resources, including support groups and community programs, to enhance their social engagement and quality of life.

• Maintain accurate and up-to-date documentation concerning assessments, care plans, progress notes, and all related interactions in accordance with organizational standards.

Qualifications\n
