Registered Nurse RN - Home Health PRNLebanon, OhioOverviewThe Home Health Registered Nurse (RN) PRN is responsible for planning, coordinating and documenting skilled professional nursing care provided to meet patient need from the day of admission through day of discharge. Utilizing physician's orders and professional skills, the RN functions as a case manager and develops and implements a Home Health plan of care that meets each patient's specific needs and is in compliance with State and Federal Home Health regulations, reimbursement guidelines, as well as Otterbein Home Health policies and procedures.The RN utilizes the nursing process which includes the use of technology, therapeutic intervention, evidence-based practice and coordination of care with other health team members to achieve positive clinical outcomes with the home health patients. The RN is responsible for the on-sight supervision of the home health aide.This position travels locally throughout the greater Lebanon Ohio area and Clermont County providing patient services in a Home Health care setting.At Otterbein, you're more than an employee, you're a Partner in Caring. Together, we work side by side toward a shared goal: delivering person-centered care that respects every resident and the choices they make.Whether in our vibrant communities, our welcoming small house neighborhoods, Home Health, Hospice or Home Office, we provide the highest level of compassionate, quality care.Join our team of Partners who are talented, kind, wise, funny, spirited, generous, endearing, and truly one-of-a-kind.Shift: PRN weekend coverageResponsibilitiesCompletes an initial assessment of patient and family to determine home care needs; Provides a comprehensive physical assessment and historyInitiates the plan of care and makes necessary revisions as patient status and needs changeUses health assessment data to determine nursing diagnosisEstablishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions; Includes the patient and the family in the planning processInitiates appropriate preventive and rehabilitative nursing procedures; Administers medications and treatments as prescribed by the physicianCounsels the patient and family in meeting nursing and related needsProvides health care instructions to the patient as appropriate per assessment and plan of careIdentifies discharge planning needs as part of the care plan development and implements prior to discharge of the patientPrepares clinical notes and updates the primary physician when necessary and at least every sixty daysCommunicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as requiredCommunicates with community health related persons to coordinate the care planParticipates in on-call duties as defined by the on-call policyEnsures that arrangements for equipment and other necessary items and services are availableInstructs, supervises and evaluates home health aide care providedReview LPN visit records and physician orders and co-signQualificationsEducation: Bachelor's degree in Nursing preferred. Graduate of approved nursing programLicensure/Certification: Valid Ohio Registered Nurse (RN) license, possesses and maintains current CPR certification.
