Job Summary:
Home Healthcare Registered Nurse (RN) in Littleton, New Hampshire. This position involves creating individualized care plans for patients receiving home health and hospice services. The RN will work independently within a collaborative, multi-disciplinary team to deliver high-quality patient care in a home setting. Job Details:

• Schedule: Day shift, 8 hours per shift, minimum of 40 hours per week.
• Required Qualifications: Valid New Hampshire nursing license, current Basic Life Support (BLS) certification from the American Heart Association, active auto liability insurance meeting specified coverage limits, current auto insurance documentation, and minimum 18 months of home health experience.
• Preferred Qualifications: Previous hospice experience, 2-5 years of med-surg experience, supervisory reference from the past year, and proficiency in electronic medical records data entry.
• Additional Information: Candidate must possess strong self-direction, flexibility, and the ability to function effectively in various home environments regardless of patient demographics or background. Must demonstrate excellent communication skills and maintain all required licenses and certifications current at the time of submission.
