Job Summary

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Coordinate and facilitate the development of treatment plans with interdisciplinary care teams.
• Assess patients' needs and manage discharge planning effectively.
• Float to different facilities within the state, including the Laguna Facility as needed.
• Attend and organize family meetings to discuss patients' care.
• Work independently to arrange Durable Medical Equipment (DME), Skilled Nursing Facility (SNF), and transport for patients.
• Maintain compliance with hospital policies and handle care management for up to 25 patients.

Qualifications

• Active CA state RN license, or pending with license upon start date.
• Minimum of 2 years' experience in acute medical care management and care coordination.
• Proficiency in Epic charting system required.
• Previous travel experience required, with a background in patient care coordination.
• Experience in trauma Level I or II facilities is preferred.
• Ability to work weekends and participate in a weekend rotation.
