Work Environment

This position is based onsite at our assisted living facility, allowing you to build meaningful relationships with patients and their families in a familiar and engaging setting. Your typical work schedule will be Monday through Friday, from 9:00 a.m. to 6:00 p.m., with the possibility of extending hours for evening clinics on occasion and participation in community outreach activities during the weekends. Each week, you can anticipate a standard workload totaling around 40 hours.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients of all ages in both individual and family contexts, ensuring personalized care plans that address unique health challenges.
• Diagnose and manage acute and chronic medical conditions, implementing effective treatment plans that integrate preventive care and health promotion strategies.
• Collaborate with local community organizations and stakeholders to develop cohesive health education programs, workshops, and initiatives aimed at promoting community well-being.
• Provide culturally competent care that respects diverse backgrounds and advocates for patients' rights, ensuring all patients feel empowered in their healthcare decisions.
• Coordinate referrals and follow-up care to maintain continuity in patient health services, utilizing electronic health record systems to track patient information efficiently.
• Document interactions and care plans meticulously in adherence to regulatory standards and best practices, ensuring compliance and quality assurance.

Patient Population / Client Focus

Your primary focus will be providing healthcare services to a diverse patient population, including older adults and their families, within an assisted living environment. You will address a wide range of health issues, from chronic conditions like diabetes and hypertension to acute illnesses and preventive health assessments, always emphasizing a family-centered approach to care.

Qualifications

• Valid certification as a Nurse Practitioner is required.
• An active state nursing license for New York is essential.
• A bachelor's and, preferably, a master's degree in nursing from an accredited program.
• At least 3 years of clinical experience is necessary, ideally within community health or public health settings.
• Exceptional interpersonal and communication skills, with the ability to establish rapport with patients from varied backgrounds.
• A demonstrated commitment to health equity, social justice, and advocacy for marginalized communities.

Preferred Experience

While a minimum of 3 years of experience is required, candidates with 5 years of experience in a community or public health role will be strongly favored. Experience managing care in assisted living facilities or similar settings is advantageous.

Compensation and Benefits

This full-time Family Nurse Practitioner position offers a competitive salary ranging from USD 180,000 to 200,000 annually, commensurate with your experience and qualifications.
