Primary Care Advanced Practice Provider (NP OR PA)

Overview This is a fully remote, full‑time, 5‑day, 8‑hour position (8:30AM–5:00PM MST/PST). Licensure as an RN, APRN, NP, or Physician Assistant is required. Responsibilities Provide virtual primary care visits via telehealth, including comprehensive assessments, chronic disease management, preventive screenings, and acute care interventions. Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD, heart disease) and develop individualized care plans. Evaluate and treat acute concerns (e.g., infections, respiratory illnesses, minor injuries, skin conditions) within the scope of virtual care. Order and interpret lab tests, imaging, and other diagnostics, coordinating follow‑up care as necessary. Manage medication prescriptions, titrations, and refills, ensuring patient adherence and safety. Provide health coaching, lifestyle modification guidance, and patient education to empower patients. Identify patients needing referrals or home‑based services and facilitate coordination with specialists or community resources. Utilize and provide feedback on the electronic health platform, contributing to continuous improvement. Participate in clinical case reviews, team huddles, and interdisciplinary collaborations. Engage in continuous learning and improvement, staying up‑to‑date with best practices in virtual care. Licensing & Certifications Active Nurse Practitioner (NP) license in good standing in existing or future Devoted States, or ability to obtain licensure in multiple states within 90days of hire. RN, APRN, or Physician Assistant licenses active and in good standing. Board certification in Family Medicine, Adult‑Gerontology, or Internal Medicine. Minimum 2years of primary care, urgent care, or internal medicine experience (preferably telehealth). Active BLS certification at time of hire. Desired Skills and Experience Experience managing chronic conditions, preventive care, and acute illnesses via telehealth. Familiarity with value‑based care models, population health management, and quality metrics (HEDIS, STAR ratings). Comfortable with digital documentation and telemedicine platforms (EHRs and virtual care workflows). Strong communication, teamwork, and patient‑centered care focus. Compensation and Benefits Base salary $120k–$155k plus performance‑based bonus $10k–$20k quarterly/annually for total compensation $120k–$170k.
