Part-Time EPSDT Registered Nurse (RN)

Looking for a flexible RN role outside of traditional bedside nursing?

Heritage Home Healthcare is seeking a Registered Nurse (RN) to provide face-to-face supervisory and supportive visits for EPSDT clients throughout the greater Albuquerque area.

This is a part-time, hourly position paying $33.00 per hour , with an assigned caseload of approximately 25 clients. The role focuses on client well-being, supervisory visits, documentation, and communication with clients, families, and caregivers.

This is not a skilled nursing position and does not involve routine skilled nursing procedures.

What You'll Do

• Conduct face-to-face supervisory visits with EPSDT clients in their homes approximately every 60 days.
• Manage an assigned caseload of approximately 25 clients.
• Assess overall client well-being and identify concerns or changes requiring follow-up.
• Communicate with clients, families, caregivers, and members of the care team.
• Provide education, guidance, and appropriate resources as needed.
• Accurately document visit findings in a timely manner.
• Communicate concerns and recommendations to the appropriate team members.
• Maintain accurate and up-to-date client records.
• Ensure visits and documentation meet applicable regulatory and organizational requirements.
• Work independently while remaining connected to a supportive clinical team.

Service Area

This position serves clients throughout the greater Albuquerque area and surrounding communities.

The majority of clients are located in Los Lunas and surrounding areas , with a smaller number of clients in more outlying communities.

The territory may extend approximately:

• North: Santo Domingo Pueblo
• East: Moriarty
• South: Socorro
• West: Grants

The overall territory is within approximately 100 miles of Albuquerque , so candidates should be comfortable with occasional longer-distance travel.

What We're Looking For

• Current, valid New Mexico RN license
• At least 1 year of nursing experience
• Home health, community health, case management, or similar experience preferred
• Strong assessment and documentation skills
• Excellent communication and interpersonal skills
• Ability to work independently and manage a flexible schedule
• Compassionate, patient-centered approach
• Valid driver's license and reliable transportation
• Comfortable traveling throughout the assigned service area
