- Actively monitor and enhance the patient visit schedule for maximum productivity and responsiveness to patient needs.
- Conduct comprehensive assessments (OASIS and PT) focusing on psychosocial, educational, safety, and risk factors in line with regulations and care standards.
- Develop personalized care plans aimed at promoting optimal functional independence and safety, with clear patient-specific goals.
- Provide skilled physical therapy treatment using various therapeutic techniques, while consistently assessing patient progress.
- Collaborate with an interdisciplinary team to ensure coordinated and effective patient care, fostering open communication with all clinical team members.
- Supervise and guide the clinical activities of PTAs, PT students, and home care aides to uphold the highest standards of care.
- Manage required paperwork to comply with regulatory and organizational policies, ensuring all documentation accurately reflects care provided.
- Execute billing and charging processes accurately, addressing any inconsistencies in insurance coverage proactively.
- Engage in training programs for colleagues and community groups to promote our services and knowledge in the field.
- Continuously enhance personal knowledge and stay abreast of physical therapy standards and best practices.
- Actively participate in marketing and promotional activities for rehabilitation services within the community.
- Conduct routine health assessments and procedures including PT/INR checks and wound care as needed.
- Communicate effectively about changes in patient care schedules with appropriate stakeholders.
- Complete OASIS time points in accordance with all relevant guidelines and regulations.
- Collaborate with Quality Improvement (QI) staff to ensure the accuracy of clinical notes and OASIS documentation.
- Follow agency protocols regarding visitation processes and documentation for optimal operational efficiency.
- Assess patient needs for home care aide services and develop coordinated plans of care, providing ongoing supervision as necessary.
- Coordinate safe patient discharges or transfers, ensuring compliance with all required documentation and procedures.
- Report any patient incidents in a timely manner to the QI Specialist.
