In your role, you will thoughtfully evaluate patients, whether they are in an inpatient or outpatient setting, to create customized treatment plans tailored to their unique needs. You will focus your interventions on enhancing mobility, strength, balance, and functional abilities, collaborating closely with nurses, physicians, case managers, and fellow rehabilitation professionals to ensure optimal outcomes. With your patient-centered approach, you'll educate familiessharing insights about therapeutic goals and effective home exercisesto create a supportive environment beyond our walls. Your responsibilities will also include meticulous documentation of evaluations, treatments, and progress in accordance with facility guidelines, alongside participating in interdisciplinary conferences to promote coordinated care across the continuum. To thrive in this role, you will need a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program, along with a state license and preferably CPR certification. Your previous experience in similar environments will be invaluable, as will your ability to engage with patients who face various complex medical and mobility challenges.
