Direct Support Worker (DSW)

Position Summary

Creative Management Source Inc. is seeking a dependable, compassionate, and energetic Direct Support Worker (DSW) to provide community-based support to adults with disabilities.

The Direct Support Worker will assist individuals in participating in meaningful community activities, developing greater independence, strengthening social and daily living skills, and accessing community resources. This position also requires providing safe and reliable transportation to approved activities, appointments, programs, and other community destinations.

We are looking for individuals who believe in treating every person with dignity, respect, patience, and compassion while encouraging independence and community inclusion.

Schedule

• Monday through Friday
• Occasional weekend availability required
• Schedule may vary based on individual and program needs
• Flexibility may be required for community activities and special events

Essential Duties and Responsibilities

• Provide direct support to adults with disabilities in community-based settings.
• Assist individuals with recreational, educational, social, and community activities.
• Provide safe and reliable transportation to and from approved activities, appointments, programs, and other community destinations.
• Encourage individuals to develop and maintain independence in daily living and community participation.
• Assist individuals with developing appropriate social, communication, and interpersonal skills.
• Support individuals in identifying and accessing community resources.
• Promote individual choice, self-advocacy, dignity, and personal decision-making.
• Maintain professional and respectful relationships with individuals, families, coworkers, and community partners.
• Follow each individual's service plan, goals, and established support strategies.
• Observe and appropriately report changes in behavior, needs, or circumstances.
• Complete required documentation accurately and within established timeframes.
• Maintain confidentiality and comply with agency policies and applicable regulations.
• Follow all health, safety, emergency, and incident-reporting procedures.
• Participate in required staff meetings, training, and professional development.
• Perform other duties related to providing quality community-based support as assigned.

Qualifications

• High school diploma or GED required.
• Experience working with adults with intellectual/developmental disabilities, behavioral health needs, or other disabilities is preferred.
• Must demonstrate patience, professionalism, reliability, and good judgment.
• Strong communication and interpersonal skills.
• Ability to work independently while following agency policies and individual service plans.
• Ability to respond appropriately to challenging situations and seek supervisory assistance when necessary.
• Must be dependable and maintain consistent attendance.

Transportation Requirements

Because transportation and community participation are essential functions of this position, applicants must:

• Possess a valid driver's license .
• Have a safe driving record that meets agency requirements.
• Be comfortable transporting individuals within the community.
• Meet applicable agency and insurance requirements for transporting individuals.
• Maintain required documentation related to transportation and community activities.

Physical and Work Requirements

The position may require walking, standing, sitting, bending, and accompanying individuals throughout various community environments. Employees must be able to perform the essential functions of the position with or without reasonable accommodation.
