Key Responsibilities\n

• Conduct thorough screenings, assessments, and triages for individuals referred by various sources, including security and healthcare staff, ensuring accurate recommendations for their mental health care needs.

• Collaborate with patients to create tailored treatment plans based on individual assessments and offer psychoeducational and transitional group services aimed at fostering skills and coping mechanisms.

• Implement crisis intervention strategies, responding to immediate needs with effective de-escalation techniques while overseeing the clinical needs of your caseload.

• Coordinate communication with prescribing providers for medication evaluations, monitoring adherence to prescribed treatment and making recommendations as necessary.

• Document all engagement and findings meticulously in patient health records, ensuring compliance with both corporate and clinic-specific policies.

• Participate in multidisciplinary team meetings to review patient progress and adapt treatment plans collaboratively.

• Train and inform personnel on relevant mental health issues and updates to enhance the overall team’s knowledge and service delivery.
