Senior RN Case Manager: Discharge & Care Coordination

Inova Fairfax Hospital is seeking a dedicated experienced Registered Nurse Case Manager 3 to join the Case Management team. The RN Case Manager develops, implements and evaluates patient care plans and coordinates discharge planning across acute and post-acute settings. You will collaborate with physicians and the care team to ensure timely, appropriate care, manage utilization and payer authorization, and actively participate in performance improvement initiatives. #J-18808-Ljbffr Inova Health System
