Advanced Practice Provider-Full-Time Intellectual and Development Disabilities (IDD) Services

  • Salary Range: $ 113,506 to 170,269/annually based on full-time work and relevant years of experience (prorated for part-time positions)
  • Location: Larimer county, Colorado
  • Status: Full-Time or Part-Time Opportunity

The salary range for this position is SummitStone’s good faith belief and reasonable estimate of the range of possible compensation for this position at the time of the posting. Role Overview:

Responsible for providing direct professional services in the evaluation and treatment of clients across multiple care settings within a community mental health system, including inpatient, outpatient, residential, community-based, and partner sites. This role supports individuals with complex behavioral health, substance use, and medical needs, including those involved in the criminal just system and those experiencing homelessness or housing instability. The Advanced Practice Provide delivers integrated care that may include psychiatric evaluation, medication management, primary care services, and care coordination, working collaboratively with multidisciplinary teams, community partners, and external systems such as courts, residential programs, and community-based service centers.

An Advanced Practice Provider (Nurse Practitioner or Physician Assistant) is needed to help us in our endeavor to build the healthiest community in the nation—one person at a time.

This position offers a unique opportunity to provide integrated behavioral health care for individuals with intellectual and developmental disabilities (IDD) while partnering closely with community agencies, families, caregivers, and multidisciplinary teams.

We are seeking a provider who is passionate about serving individuals with IDD and who will act as both a clinical resource and an advocate for this population. The ideal candidate understands the unique healthcare challenges faced by individuals with intellectual and developmental disabilities and is committed to promoting dignity, inclusion, access to care, and improved health outcomes.

Essential Duties:

  • Evaluate, diagnose, and treat individuals with mental health, substance use, and co-occurring medical conditions
  • Provide psychiatric evaluation, medication management, and primary care services within scope of practice
  • Perform physical assessments, review medical histories, interpret clinical data, and complete procedures such as injections, wound care, and other minor medical interventions
  • Manage acute and chronic conditions, including crisis intervention and emergency stabilization for withdrawal, intoxication, or other acute medical conditions
  • Participate as the medical representative within multidisciplinary treatment teams to support integrated care
  • Develop, implement, and monitor individualized, person-centered treatment plans
  • Educate and counsel individuals on treatment plans, medication adherence, and overall health and wellness
  • Coordinate care and support continuity across levels of care, providers, and service systems
  • Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory standards and adhere to all organizational policies, clinical standards, and applicable state and federal regulations
  • Participate in on-call responsibilities, travel between service locations and partner organizations, and work across inpatient, outpatient, residential, and community-based settings as needed to support service delivery and organizational operations
  • Perform other duties as assigned.

Minimum Qualifications

  • Master’s degree from an accredited program in Advanced Practice Nursing (APN) or Physician Assistant (PA) studies required
  • Minimum of four (4) years of experience in a related clinical field required

Requirements for Physician Assistants (PA)

  • Master's degree in physician assistant studies
  • Active, unrestricted license to practice as a Physician Assistant in the State of Colorado
  • National certification as a Physician Assistant (PA-C) issued by the National Commission on Certification of Physician Assistants (NCCPA), maintained throughout employment

Requirements for Nurse Practitioners (NP)

  • Master's degree in nursing from an accredited Nurse Practitioner program
  • Active, unrestricted Nurse Practitioner license in the State of Colorado (or compact privileges, if applicable)
  • Active, unrestricted Registered Nurse (RN) license in the State of Colorado, or ability to obtain within 30 days of hire
  • National certification as a Nurse Practitioner, including Family Nurse Practitioner (FNP) or Psychiatric Mental Health Nurse Practitioner (PMHNP), issued by ANCC or AANPCB and maintained throughout employment
  • Full or provisional prescriptive authority eligibility; must obtain prescriptive authority as a condition of employment
  • Prescriptive authority in accordance with state regulations and organizational privileging processes

Licensure, Certification, and Credentialing

  • Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross, maintained throughout employment
  • Valid Colorado driver’s license (or ability to obtain within 30 days of hire) and a clean driving record
  • Unrestricted Drug Enforcement Administration (DEA) registration with authority to prescribe medications within scope of practice
  • Eligible to provide reimbursable services and maintain active provider status with Medicare and Medicaid
  • No sanctions or exclusions from the U.S. Department of Health and Human Services Office of Inspector General (HHS/OIG) or General Services Administration (GSA) exclusion lists

Preferred Qualifications

  • Two (2) or more years of experience treating individuals with substance use and mental health conditions preferred
  • Experience working with individuals with intellectual and developmental disabilities.
  • Experience in community mental health, behavioral health, psychiatry, or integrated care settings.
  • Knowledge of person-centered planning principles.
  • Experience collaborating with community agencies and support systems.
  • Spanish language skills or other bilingual capabilities.
  • Valid Colorado driver's license if position responsibilities require travel between community locations.
