LTSS Service Care Manager

Immediate need for a talented LTSS Service Care Manager. This is a 06+months contract opportunity with long-term potential and is located in U.S(Remote). Please review the job description below and contact me ASAP if you are interested.

Pay Range: $26/hr - $29/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Manage a caseload of 50-70 members, focusing on geriatric long-term care.
• Conduct comprehensive assessments with members, caregivers, and providers to develop care plans.
• Monitor service delivery and follow up through in-person visits and telephonic contact.
• Authorize and coordinate referrals for necessary services, ensuring no gaps in care.
• Collaborate with discharge planners, physicians, and other stakeholders to facilitate effective care transitions.
• Assist members in filing and resolving complaints and appeals.

Key Requirements and Technology Experience:

• 2+ years of Care Management experience, with a strong emphasis on field experience.
• Proven ability to manage high caseloads in a fast-paced environment.
• Experience with Medicaid and Medicare, particularly in long-term care settings.
• Proficiency in electronic medical health records and technology-savvy skills.
• Valid Driver's License.
• Bachelor's Degree in a related field.
• Home Health experience.
