Field LTSS Care Manager - Waiver & Home Visits

Molina Healthcare is seeking an experienced Care Manager to join our LTSS team serving Medicaid members in Northern Cook County. The role involves face-to-face home visits and collaboration with a multidisciplinary team to coordinate long-term services and supports. Ideal candidates have 2+ years in health care, LTSS or care management, and hold a bachelor’s or master’s degree in a health care field.
