Field Care Manager - LTSS & Home-Visit Specialist

Molina Healthcare in Northern Cook County, IL, seeks an experienced Care Manager (Bachelor's or Master's trained) to join our LTSS team serving Medicaid members north of Berwyn and Cicero. Face-to-face home visits and local travel are part of the role. You will coordinate care with a multidisciplinary team, develop and monitor care plans, and ensure services align with member goals while maintaining regulatory documentation.
