Key Responsibilities\n

• Utilize your education and clinical training to independently assess and diagnose patients’ psychosocial conditions.

• Engage with patients collaboratively to develop individualized treatment plans that prioritize their specific needs and goals.

• Implement evidence-based behavioral interventions, such as solution-focused therapy, motivational interviewing, and behavioral activation, to address a wide range of mental health issues.

• Work closely with multidisciplinary teams including psychologists and primary care providers to coordinate comprehensive patient care, ensuring that all aspects of a patient’s health are addressed.

• Monitor patient progress through systematic tracking of treatment responses, utilizing both virtual and in-person consultations as appropriate.

• Assist with psychoactive medication management, working to ensure adherence to prescribed treatments while closely monitoring side effects and overall effectiveness.

Patient Population / Client Focus\n

This role primarily focuses on providing support to patients within a hospice setting. You will work with individuals facing end-of-life challenges, offering guidance not only to the patients but also to their families. Qualifications\n

• Master’s degree in Social Work from an accredited program.

• Possession of an active and unrestricted LCSW license to practice independently in the state of New York.
