Your Impactful Role:
As a Social Worker at Greenfield Healthcare & Rehab, you will play a vital role in promoting the well-being of our residents by addressing their physical, emotional, and psychosocial needs.

Responsibilities:

• Support residents in maintaining and improving their daily physical and emotional well-being
• Assess and identify individual social and emotional needs
• Develop and maintain personalized care plans for residents
• Coordinate residents' admissions, community re-entries, or transfers to other facilities
• Participate in interdisciplinary care plan meetings to ensure a holistic approach to resident care
• Collaborate with the finance team to educate residents and complete necessary documentation
• Plan and coordinate discharge processes, including arranging DME (Durable Medical Equipment) and home care, ensuring residents' safety when returning to the community

What We’re Looking For:

• Education: BSW or MSW preferred, or a Bachelor’s degree in a related field
• Experience: Minimum of 1 year in Social Work; experience in long-term care or sub-acute settings is a plus
• Skills: Strong interpersonal, organizational, and problem-solving skills
