Schedule / Job Type

Contract | Days | Five 8-hour shifts per week | Rotating weekend schedule (8am–5pm) | Holiday rotation required

Key Responsibilities

• Provide home health visits covering Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily visits — targeting approximately 6.5 points per day
• Treat patients with diagnoses including CHF, orthopedic conditions, and pneumonia in a home health setting
• Utilize Home Care Home Base (HCHB) software to document and manage patient care
• Operate independently in the field while maintaining communication with the clinical manager or educator, available 24/7 for emergencies and 8am–5pm for general support
• Adhere to dress code requirements (scrubs or business casual)

Required Qualifications

• Active PTA license required — must be in hand at time of submission
• BLS certification required
• Prior home health experience required
• Prior experience with Home Care Home Base (HCHB) required

Preferred Experience / Skills

• Experience with SOC, ROC, recertification, and evaluation visits preferred
• Candidates with prior travel or contract experience preferred; new graduates are discouraged
