HCBS Case Manager, Day Treatment

Position Summary Provide all case management duties related to authorization and re‑authorization for clients in Day Treatment services. Include but not limited to CFT, PCP development, SAR submission, etc. Qualifications Education: Bachelor’s Degree in Human Services Field. Work Experience: Minimum two (2) years of documented clinical experience working with the population served. Knowledge, Skills & Abilities: Knowledge of community resources; written and verbal communication skills; computer proficiency (word processing, Excel, e‑notes, photocopier, fax machine); organizational and prioritization skills; flexibility; multitasking; relationship building; sound judgment; takes direction well and demonstrates initiative. Travel Requirements: Service delivered in school setting. Essential Job Duties Complete all CFTs, PCPs and submit authorizations for clients in Day Treatment services – 50% Actively participate in development of Person‑Centered Plans with school personnel, social services professionals, assigned client(s) and their families; maintain up‑to‑date client records following YH standards; prepare necessary reports and file documents timely and accurately – 25% Complete required documentation in a timely fashion – 10% Attend sites for CFT meetings and meet with Site Managers to discuss client progress – 10% Other duties as assigned/required – 5% Equal Employment Opportunity Youth Haven Services, LLC is committed to the principle of Equal Employment Opportunity for all employees and applicants.
