Minimum 3+ years recent Case Management / Care Coordination RN experience (acute care required) / CERTIFICATIONS – BLS required / Schedule: Days 8hr shifts, 40 hrs/week, 13 weeks, Every other weekend, First to Float / Start Date: 04/28/2026 / Unit: Care Coordination (Case Management) Patient Ratio: 1:20
Skills Required: Discharge planning, utilization review, acute care coordination, working with placement agencies, Medicare/Medi-Cal guidelines, managing socially complex discharges / Proficiency in Midas, InterCall, Cerner, Curaspan, Microsoft Office
Experience Required: Minimum 3 years hospital experience + prior CA Dignity/CommonSpirit acute care experience (MANDATORY)
EMR: Cerner / Floating: All adult units (excluding women’s health if no experience)PAY Negotiable.
