How you’ll make an impact in this role

• Strategic Partnership: Serve as the primary liaison between administration, physicians, and managed care partners.

• Care Coordination: Direct treatment efforts while championing quality improvement and staff education.

• Data Synthesis: Compile metrics and track outcomes to drive high-level process improvements.

• Patient Advocacy: Manage specialized referrals and follow-up care for uninsured and under-insured populations.

• Community Engagement: Foster vital relationships with community organizations to extend the continuum of care.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
